2023-24 Required Documents to Accept Seat

Documents you will need to provide within FIVE (5) SCHOOL DAYS of a seat offer.

Proof of residency does not apply to homeless students and families covered under the McKinney-Vento Act. All homeless families should contact the AMSA's McKinney-Vento liaison,mckinneyvento@amsacs.org who will work with you to enroll your students.

Proof of Residency #1

You will need one of the documents below. The name of the primary parent/guardian who made the child's application must be on the document.

  • Most recent mortgage payment (dated within 30 days).
  • Original, current Lease (including HUD leases) signed by property owner/management company and tenant. Must include names of all occupants, and record of most recent rent payment. Original, current lease must accompany all addendums for extending lease terms.
  • Property Deed. Evidence of mortgage payoff must accompany this document.
  • Section 8 Agreement.

Proof of Residency #2

You will need one of the documents below. The name of the parent/guardian who made the child's application must be on the document.

  • Valid MA Driver's License/ Valid MA Photo ID Card/ MA Real ID Card
  • Current W2
  • Current Excise (vehicle) Tax Bill
  • Payroll Stub (dated within last 30 days)
  • Bank or Credit Card Statement (dated within the last 30 days)
  • Certificate of Residency from Town Hall
  • Letter from approved government agency (dated within last 30 days)

Proof of Residency #3

You will need one of the documents below. Each proof must be of a different type to the one used in Proof of Residency #2, ie, not another parent Driver's License or another credit card statement.

  • A recent (within 60 days) Gas/Water/Oil/Electricity/Cable Water bill - Cell Phone bill is not acceptable
  • Letter from an approved government agency (dated within last 30 days)
  • Certificate of Residency from Town Hall
  • Current W2
  • Current Excise(vehicle) Tax Bill
  • Payroll Stub (dated within last 30 days)
  • Bank or Credit card Statement showing address and date within last 30 days

Student Report Card

You will need a recent copy of your student's report card as proof of CURRENT school and GRADE level eligibility. 

The document should show the School Year 2022-23 (or a date within this school year), current school, grade in school along with the child's grades in classes. You might need to combine pages into one file to upload if they are screenshots of a student's electronic portal. If you are homeschooling this year submit the latest progress as submitted to your school district. Email the letter authorizing your homeschooling to Admissions@amsacs.org with your child's Last name and ID Number in the subject line. If your child is attending a public school outside their town/city of residence please email the school choice documentation to Admissions@amsacs.org.

Birth Certificate

You will need your child's birth certificate. If it is in a language other than English, please include an official translation.
